Background: The most common complication after hemorrhoidectomy is almost always postoperative. This dreadful makes the patients postpone treatment even for prolapsing, bleeding, and uncomfortable piles. There are different methods that can be used reducing pain including performing a lateral internal sphincterotomy (LIS). Better outcome of combined with has been observed in few past controlled trials respect to decreased surgery, improved early wound healing. Objective: To compare mean post-operative score undergoing open without III IV degree hemorrhoids. Study Design: Randomized control trial. Place Duration Study: Department Surgery, Jinnah Hospital Lahore from 1st July 2021 31st December 2021. Methodology: One hundred twenty diagnosis & hemorrhoids were selected. They divided two groups, Group A included who underwent B alone. Standard Milligan Morgan Hemorrhoidectomy was done under local anesthesia. fine surgical blade inserted inter-sphincteric groove parallel circular fibers sphincter. Blade then rotated so it faces lumen anal canal. With surgeon’s index finger canal, advanced towards as divide sphincter but not mucosa. Post-operative assessed at first 48 hours post operatively according ‘Visual Analog Score’. Results: age 34.6±7.72 34.28±8.09 years B. 34 (56.67%) males 32 (53.33%) while 26 (43.33%) females 28 (46.67%) Group-B. 1.43±0.49 2.03±0.74 (P=0.001).
